I have found that it is hit or miss. I have seen them replace stuff that they shouldn't, like a pair that had been run over by a jeep. Really what is covered is coatings on lenses, rubber on frames, hinges on frame, and the like. I am sure they get alot of folks sending in lenses that are cracked from dropping them or examples like the jeep running over them. Technically that is not covered under warranty but I have seen those replaced under warranty too. My opticians always tell folks it is worth the $11 to see what they will do. Worst case they tell you it is not a warranty defect and they offer you a new pair at dealer cost. At least that is our experience. They do not require a proof of purchase on warranty just a date when purchased.
Maui will make you send in a proof of purchase and if it is a day outside of 2 years it is a no go. They are more strict on the manufactures defect than costa as well.
This was also my second bad experience. The first was just as bad. Like i said, i love the glasses. Best i've ever had but i couldn't afford to buy a new pair every two years.

Never had pair of Jims but have had too many of others .. Costa are good and have great marketing when they went public to keep up with Ocean Waves which is still private.. OW have the best glass I have ever had in pair shades and great CS.. they are the preferred glass for most of the bigger time fishing guides that I ran into in the 8 yrs I lived down in St Pete area.

previous pair were maui jim ho'ookipa, current pair are costa permit 580 (vents don't work as well as expected), next pair will be maui jims...when costa eventually refuses warranty coverage (infused rubber has separated on 2 pair that were replaced and i went ahead and bought new lenses for $50 last time, so won't be anytime soon) costas' frames can take more abuse, but the maui jim's light weight with comparable clarity and crispness of lenses is really nice!
like costa, maui pricing is pretty standard across the board unless you find a discontinued model

Jim - I will be picking up a pair of Costa's Monday. Going with the gray glass lenses in Jose model. I'm looking forward to getting them.

Picked I some Maui my sport's today. Very lightweight and the glass was as good as the 580's I was comparing them with.
